The Twelve Tribe's of Israel Identity & Origin

I have always thought that the 12 tribes are just that, 12 separate groups. Due to changes in social events, whether it be captivity or other reasons, the tribes have scattered from Africa, Asia and Europe. Jews equal Judah. I've always wondered at what point Israel, a nation of 12 tribes became Isreal with everyone called Jews from the tribe of Judah and the tribe name changed to a religion or religious people like Catholics.
